Revised: Knight Cancer Institute & Clinical Consent and Authorization Form Templates
Updated consent and authorization form (ICF) templates are available for Knight Cancer Institute and clinical research effective June 1, 2024. Revisions include substantive language updates as well as header and sub-header formatting updates for improved accessibility. Researchers who have started a protocol with a previous version of the consent template are not required to update to the new version. Previous versions of the templates will be accepted until July 31, 2024. After that date, the new templates must be used for protocol submissions.
